uninsured adults. Our goal is to offer comprehensive
care by not only providing for physical needs but also
for the spiritual and emotional concerns of this area.

people and the community as an act of Christian

| ELIGIBILITY |
The governing board of the CHCB has established that those eligible for care in the clinic include individuals whose income is at or below 150% of the poverty level, between the ages of 19 and 64, a U.S. Citizen, who are not eligible for Medicare or Medicaid, and who do not currently have medical insurance. |

We will provide services to those who meet these requirements regardless of sex, race or religion. An $8.00 fee is charged to all clinic patients. This will be used to help defray operational and pharmaceutical costs of the clinic. There is a $5.00 fee for prescription refills on medications prescribed by a CHCB physician. This is the total fee regardless of how many you need refilled |
